Respite and Summer Camp Programming Application
Criteria for Respite Program:
• Participants must be 8 years of age or older and have a documented developmental disability. • Participants must live at home with family or legal guardian. Individuals living in group homes or other residential facility do not qualify for this program. • Participants must live in one of the seven counties of Western New York (Allegany, Cattaraugus, Chautauqua, Erie, Genesee, Niagara, Orleans) • Participants must have a physical examination, current medication orders, and prescription for and physician approved over the counter medication form (OTC Form) within a year of their respite weekend.  |
